Every child and young person that accesses Bridge Together must be registered with us so we have all of the correct and appropriate information to keep your child safe and support their needs.

If your child has medical needs and will need to take medication whilst attending Bridge Together, then you will also need to complete the Medication Consent Form.

All information is kept securely within Bridge Together and will only be shared with the necessary Bridge Together staff or with other relevant professionals. All forms are reviewed annually.
